I think its the fat toes not the toenail so I guess u have to trim some fat off.

  I had a procedure like this done when I was younger. I would constantly get ingrown nails on my big toe...after multiple nail extractions, they actually took the top layer of one side of the big toe skin off (not as drastic as the video and only one side). My toe looks perfectly normal and I've never had an ingrown toenail since (12 yrs).
